Local Upgrade leads to Non Bootable drive 1

Hey all, my Mitel VAR came onsite for an unrelated problem, but gave me the software for V5.x.x.x.

I have no intention installing this on the work system, but thought it would be ok to install on my home can.

Well things did not goes as planned, I can no longer boot off the drive in the SX200-ICP.

I used "local" for the upgrade. Here are the errors I am getting:

The controller just blinks with all alarm lights blinking. If you need more info just ask and i'll try to get you the relevant reports

Hmmm one other thing, I have never seen the flash card LED go green always orange.
