IPO SE 12.0/12.1 Routes getting replaced with erroneous default routes on reboot 1

Avaya has issued a critical patch for this major issue, which could lead to you being locked out of your server or severe traffic misrouting issues Contact Avaya (or me) for details.
 
I see three pages of fixes in the Global IP Office Technical Bulletin R12.1.0.1.0v2.pdf but nothing mentioning the LAN2 default route issue.
 
Based on my conversation with Avaya support, it is not fixed in any of those releases. There is a separate critical patch, and apparently the critical patch will only work on a certain version of the software, which I don’t think is the current version. If that makes sense. Let me know if you want me to attach the documentation I received from Avaya

I am experiencing that now with a new installation of R12.1. After a reboot we lose connectivity to the server. We have to use a keyboard and monitor to change the IP back. Log in as root and enter #sudo ifconfig eth0 255.255.255.0 10.2.205.13 up then reboot
I just upgraded to 12.1.1 and it didn't fix the issue.
I did notice that on the web management. (7071) under settings. The LAN 1 interface and LAN 2 interface are both set to eth0.
Normally it is eth0 for Lan 1 and eth1 for Lan 2
The drop down for Lan 2 doesn't offer eth1, only eth2 and none. I set it to eth2 and save it. DId a test reboot and the Lan 1 interface held. For now.

There are two patch files.
VERY IMPORTANT - CAN ONLY BE LOADED ON 12.1.0.0.0 (WHICH IS NOT THE MOST CURRENT VERSION)
IF YOU TRY TO LOAD ONTO CURRENT VERSION YOU WILL BE TOTALLY SCREWED
ALSO NOT THIS PATCH IS NOT INCL. IN THE CURRENT VERSION!

Is this on HyperV? I see there is an issue with one one Ethernet interface configure on HyperV
 
Our issue was on physical Dell server. Not virtualized. Dell R240.
