IPO BM12 consoles

I have a Avaya IPO Standard, software 9.1.700.163 with 70 9508 phones. 4 of the phones have 3 BM12 consoles hooked up to them. On all 4 phones, the last 2 consoles will randomly lose their display. If you reset the entire phone, they will NOT come back on. You must only unplug the cord from console 1 to console 2, then the last 2 consoles will start working again. This happened 3 times in an ½ hour span. It seemed like you would reset the consoles on 1 phone, and it would cause another phones consoles to go out. Any ideas?

Do you use the power supply 1151C1 ?

I upgraded to 9.9.11 and still have the same issue. When the phone is plugged in, the 2nd and 3rd consoles do not come on. the only way to get them to work is to unplug the cord from the 1st console to the 2nd console, but they will go out 4 to 5 times a day. Any ideas?
