ipo basic thinks co lines out of service

IP office basic mode 9.0.6.979. 3 co lines from TW Cable, 1 POTS. (8) 9508 phones. Customer can be offhook, using one or two lines and the KSU randomly thinks all lines are out of service, and knocks everyone off, and then shows all lines out of service. Can hook in with butt set and dial tone is present? Does no good to disconnect CO lines and replug, only way to restore is reboot KSU. Have replaced control unit, combo card, software, and now have replaced the entire system with new. Runs ok for any where from month to only 2 days, and has happened twice in same day. Everything else on system is fine. Can intercom and page when co's are locked. Any ideas? I am thinking TWC modem
 
It is no doubt TW not giving a strong enough signal or possibly even too strong a signal. I had a client with a similar issue that every once in a while they could not access their lines. A test set always worked before the IPO. This was RCN cable, but the tech came out and measured their signal and they were giving way too much signal from the street causing the IPO to occasionally lock up the lines and drop them all. I would have TW test their signaling from their router.
 
Thanks for the response, this has been driving me crazy since January. Couldn't check much cause customer would cold start the system each time and wipe out data just to get phones back on again. I have been checking periodically with SS and notice the TWC lines lose service at odd times of day and night, all 3 but usually return to service, but wasn't sure why the POTS line would lock out too? Having the customer get TWC on it.
