IPO 500 V2 Phone 30 Flashing Red led 1

We are trying to install new phone 30 module to the existing ipo 500v2 setup, but after installing the Expansion unit phone 30 it came up for some time, but now it is continuously flashing the red led, what can be the issue.

In order of most likely:
Unit cannot find the required upgrade files on the file server
Unit is not supported in the IP Office release used
Bad cable between IP500 and unit

You did power the expansion 30 module 1st correct?

Depending upon on what release your IP500V2 is, some of those older Dig 30's needed a step through to the current release. If you tried going directly it might have bricked the module.

OK Yes, we power on the Expansion First , thought was just plug and play, but after rebooting the whole system it was working, but suddenly it started flashing red led, what need to be done to get it work again

The updated DS16/30B2 digital station modules have now started to be
provisioned by Avaya tools – a supply chain transition is occurring from the older
to the newer version of the digital station module for IP Office 9.1 and later.
The updated DS16/30B2 digital station modules are supported on
any version
of IP Office 10.1

minimum of IP Office 9.1 Service Pack 12 or IP Office 10.0
Service Pack 5 is required to support the updated
DS16/30B2
