IP Office Upgrade

I have an IP Office 406 w/4.0[7] software and I am trying to upgrade to 4.1[12] and when I try it comes back with " ALL SELECT UNITS FAILED TO TRANSFER " I press ok and it comes back with " ONE OR MORE SYSTEMS FAILED TO UPGRADE " why won't it upgrade am I missing a step , I tried on another switch and the same thing happens any tips

if it hasn't been changed, use the password (password) the system password and the configuration password are 2 separate entities
