We have an IP Office 406V2. It was installed in February. Since then, on three different occasions, we have had occurences where we had to reboot the system. We have mostly 5410s, with 3 answer stations that have 4424's with 24 button DSS. The problem first shows up on the 4424's. The lamps on the DSS start giving faulty indication....showing someone on a phone when they aren't, showing incoming lines flashing when they shouldn't be. After that, the 4424 will ring for an incoming call we answer, but we can't communicate with the person calling. This all starts happening on one 4424 then spreads to all 3. Once the 4424's are all useless in this mode, the 5410's start to fail in no particular order. If someone is on a call, the phone will stay active, but phones that aren't in use will die....no dial tone, no voicemail access, the display starts to show a timer for some reason. The only way to restore phone use is to reboot. Once that is done, we are okay for a few weeks.
When we started getting hints that it was happening again last Friday, I ran the monitor trace and have sent the report to my vendor. My question is....has anyone else seen this kind of failure ? We like the IP Office system overall, but continued intermittent failures like this were not what we were hoping for. I have 10 people who make their living on the phone, and trying to get the ones who are still on functioning phones to hang up while the others are breathing down my neck is a hassle I want to live without.
