IP 400 Digital Sation ports dying 1

We have an IPO 412 3.0(69) with 7 IP400 Digital Station units and 44XX phones. For a few months now ports on the Digital Stations randomly become unusable after a reboot. At first we were able to reuse them after a reboot but no more. The message light just flashes and incoming calls are sent to VM. I'm starting to run out of usable ports.

Any ideas?
 
how is the room where the IPO is ?
is it cooled with an airco ?
is hit hot or dusty ?
all are of influence
how old is the system ?
is it grounded well ?
sometimes ports just die
probably you need to buy new modules
 
I have this same issue w/ various systems all the way to 3.1.65. Same thing you described w/ the MWI flashing. For now, my work-around is the following:

Unplug the power and repower ONLY the module that is affected.

So the entire system is up and running, unplug the power and repower the expansion module(s) w/ the sleepy sets and all phones will come up nice.
 
I have a customer with a similar problem, they were on 3.0.69, albeit with 54xx phones. 3 different ports on 3 differed DS30 modules in the past 10 months. The recommend was to upgrade to 3.2, which I did last night. I'm not so sure of this solution as it seems like a software fix for a hardware problem. Time will tell.

- It's a proper server room with elevated floor
- yes there is airco
- there were renovations a long while back. The equipment was covered as well as we could. The symptoms only started happening about a year after that. It is also affecting a brand new IP400 added about a month ago
- The system is almost 2 years old
- The electrical system was redone at the same time as the system went in and is fully grounded

 
I havent had an issue related to tech tip 143. The modules dont reboot constantly, when a scheduled reboot is performed, some DS phones go to sleep. The last customer I dealt with this issue on is using DS16 modules.
 
Where would I find tech tip 143?
 
This was a known issue on 3.0 and the first releases of 3.1. I believe its been fixed in the most latest release (is that 3.1.65)?

Tech Tip 143 does not apply since it does not randomly reboot (although it did a while back, I may be freeing up some ports). I will be trying thephonemn's solution of unplugging an affected DS in a few minutes.

Had similar issue and it is related to the external power supplies being underpowered for digital stations that are comletely full. We contacted Catalyst and they contacted Avaya support and overnighted us aphysically larger external power supply and the problem went away.
 
VICTORY!!! thephonemn's solution worked. We unplugged the power supply from the unit, waited a few seconds and plugged it back in. The port did not re-enable itself right away. We waited 5 minutes and voila! it was fixed! We did this with all the other Expansion Units and recovered all our unusable ports.

I will be keeping an eye on spontaneous reboots. It's great if Avaya takes responsibility for their mistakes by shipping proper power supplies for their product.
