Internal Transfered calls being dropped on 6.1XC MICS

Have a MICS 6.1XC with PRI.
About 10% of the calls are being lost when transfered from the receptionist to an internal extension.

It happens on SAME receptionist phone to different extensons.
It has been confirmed it is not user error.
Are there settings that possibly could effect this either with the circuit OR switch. I do not believe that the receptionist phone has been switched out that was going to be my next step.
 
any errors on the PRI?
probably the calls are getting dropped

Do you have the latest 6.1? (SP ending in G10)
 
The software is acctually 6.0 - 30BwE02 NAT
I found a few Network Event Logs that all happened yesterday. I spoke with the customer today and they have not had any reported troubles. If you have any information it would be greatly appreciated.

Try replacing the cable from the telco d mark to the pri csu, use a cat5 cable, this has fixed the errors before, especially alarm 75.

6.1 ver 6.10 = good

We had the carrier bounce the T1 and we have had no problems for 3 days. The patch cable was changed out a while ago. If the problem persists we will upgrade the software.
