Help! Default Allocated Answer Broken

I have a 401 NG small office system connected to a 406 via VPN. We use centralized VM and when a call comes into this location and you enter an extension we get a ring no answer (the actual phone does ring forever). Internally I can dial the extension and it will roll over to VM with no problem. If I call the 401 and enter an extension on the 406 it also works flawlessly. If I dial directly into the 406 and dial an extension on the 401, it also works. The only time I have the problem is when the call is incoming to the 401. The users are complaining g that calls never roll over to VM.

By the way assisted transfer in VM pro solves the problem, but not the desired solution. This problem started 3.0.48.

Let me update this, it is only when i transfer from VMpro over the SCN. The exetnsions themselves can call each other and leave voicemail without issue. Again, if I use assisted transfer it works fine.
 
I have seen this on another site. Lodge the fault with avaya
 
what is the general approach for a loggging an issue with avaya.
