G650 Redundant Power Problem

I just installed a redundant power supply in a G650 cabinet and have the following errors/alarms. Can't quite make out what the manual is telling me to do.

Is this just a bad power supply? Or, did I not do something proper in the installation.

Install:

Hot plug
Connect cable
Walk away....
 
Did you ever find a solution to this? We are experiencing the same issue.
 
Guys I remember seeing a PCN on some of those. If I remember correctly some of the earlier power supplies came from Canada and they were junk. Anyway, I would check the Avaya support site on them.

Mine turned out to be JUNK, right outta the box! It was refurbed, supposedly tested, but it was junk.
 
Pop If I remember right the new ones (one out of three) would be bad out of the box. They started getting them from somewhere else as a fix. The serial numbers were the way you could tell where they were made.
