Extensions not working for incoming

IP Office 406v2 with 2 DS16's and 1 DS30 running version 4.0.

When extension is plugged into the 2nd DS16 it does not accept incoming calls but can make outgoing calls. Replaced the DS16 with no change.

Changed the physical extension to the DS30 and now it works fine.
 
does the frim ware on the ds16 match the 406. you may have a bad expansion port on the 406. you could try swaoing the working 16 with no working and see if it works then.

Hi, i work with Joe on this. Firmware is all up to date on all the modules. This is a very strange issue. I just swapped out the control unit and the issue still exisits. Also tried changing the stack cables with new ones. Any ideas?

try the config on a test system, could be corruption.

Or try defaulting and see if it works.

well changing the 406v2 itself would be the equiv of trying my config on a test system.

what is the best way to default?
