I have a client with the same issue. When on speakerphone it says incorrect password. When on the handset it works every time. Same R11.0.41 as you are working with. I'm guessing it is a glitch. Most of our clients use visual voicemail or vm to email so I bet this is an issue that effects more systems then they know about.
I have this issue on a few 9.1 systems, always with 1416 phones. Just a guess but it could be a combination of the room acoustics and possibly the firmware level. Not sure if an eventual updated firmware level may help with this. I had the issue in my own house in the kitchen and ended up doing what TouchToneTommy said and muted each time. If you are facing end users in your location, you might suggest the muting idea referencing room acoustics, or, the Visual Voice method. Visual took this old timer a while to get used to since I have been dialing in 770 or 777 since the early 90's, but Visual Voice allows for a little more flexibility when forwarding a message as well as offering the ability to toggle voicemail to email settings.
This only seems to be an issue for us with 9508's, not having the issue on my J100's
We are also on the same version of R11.0.4.1 Build 11
Unfortunately the system is currently deployed at the customers site. I'm sure we will be going back at some point so I will try some of the suggestions and report back. Appreciate all the suggestions.
