echo on IP 500 digital sets

Does anyone have a suggestion regarding "echoing" on digital 5400 series phone sets? Customer states a few phones are reporting this echo problem. When customer takes inbound call and says something it is being repeated while the inbound caller is talking.
Would this be a matter of a firmware upgrade? The IP 500 is on 5.0
 
The trunks are not analog. The incoming trunk lines are associated through a PRI. The trunk lines are being fed from an offsite location through the customers fiber. This fiber is pushed out to their current business location to drop off the trunks into the IPO.

Can you elaborate on this, sounds odd to me and probably the source of the echo :)

From the customer: "Some of the people here are complaining that when they are talking to a member on the phone they get what I call an echo. (they can hear their own voices on the line a moment after they talk) The echo talks over the person on the other end making it hard to hear them sometimes. Is that an issue with the phone or something else?"

The PRI feed is at the customer's old location of business which is pushing the PRI out to the new location by customer's own fiber. The CO provider was having issues through the copper setup a few years ago keeping connectivity. Thus, is the reason behind the PRI feeding from customer's old location over their own fiber to the IPO.
