Delayed dialtone on analog stations CM 8.1 and MM711 G350 1

I am having some trouble with an intermittent delayed dialtone on analog stations. I can usually figure out these types of issues, but I have gotten stuck. Any help or ideas is greatly appreciated.

Intermittently, when going off hook on an analog station dial tone is not heard for some time. I can hear several "CPC" (open loop disconnect or voltage drop) sounds in between a few seconds before finally getting dialtone and being able to place a call. This seems to occur the first time you go off hook after being on-hook for some time and then will reoccur after several times of going off and on hook. There is no issue receiving calls. The station always rings and there is no delay when answering. I've tried multiple analog phones. The CM and gateway are on the same LAN with no network trouble between the two. There are no issues with DCP phones.

A 'list trace station' shows denial event 1644 when this occurs. In the below example, the trace was started at 09:53:20. The analog station went off hook at 09:53:22 and the denial event 1644 was displayed. Dialtone was not heard until the 'active station' event at 09:53:32. The station went on-hook at 09:53:41.

At first I thought I had a faulty G350 gateway as this was happening with two different MM711 modules and the analog ports built in to the G350. I swapped out the G350 and the issue persisted. I registered the G350 to another old CM (2.1) and the analog ports work fine. I think I have narrowed this down to either a translation issue or bug in CM 8.1.

I am a novice with these systems and might be missing something simple. Below is the configuration I think might be relevant relevant. I am looking for any ideas to try. Can anyone think of what I should try next?

Have you tried creating a new extension (not duplicated)? Seems odd that the denial event is for invalid digits. There isn't a hotline setup on the station either?

I should have mentioned that this does occur with a butt set connected with a short wire to the MM711/built in ports and it does occur after creating new stations at different ports. I also thought that maybe a rotary digit was being detected so I tried turning on the "Ignore Rotary Digits" option.

There is nothing filled in for hotline designation. The denial event is odd. And it occurs immediately after going off hook, not after a timer expires. The issue is intermittent in the since that if I continually go on an off hook, it will only happen some times, but if I leave the station on hook for some amount of time, it always happens the first time I go off hook.

I just tried adding a hotline and I still get the 1644 denial event immediately after going off hook, but now if I stay off hook I never get dialtone (as expected) and the hotline call never completes. If I go on and off hook again the hotline call will complete and if I keep doing it, I will intermittently get the denial event again with indefinite dead air until I hang up.

I really hope this is some setting I can find or at least something I can correct.
 
The only thing that jumps out in my mind is once upon a time we changed some phones out instead of mucking with timers. The phones going off hook would do like a hookswitch flash.

Maybe if the port is perceiving it as the hookswitch tapping a bit, it might interpret it as a rotary dialed digit. Just a guess.
 
Ha. We were thinking the same thing. What if you turn off hookswitch flash? Can you prove it with a buttset?
 
I just tried turning off the hookswitch flash and I get the same behavior.
 
Suppose it could be a bug. For laughs, try changing the set type to another analog type (2410,6210,modem,fax).
 
It won't let me use 2410 without changing to another port. I think that might be a DCP phone? I tried 6210 and was able to save, but the issue persists. I don't have modem or fax. I think those were aliases for 2500 in previous CM releases and I don't have those aliases in this CM. I also tried K2500 and 500 and no difference.
