Dead port on a IP500 16 DS 1

A port died on the IP 500 16 DS. I swapped the phone to another module - a IP 500 16 DS and within a day the port died again.
I swapped the phone and wire.

Any ideas?

 
Have you tried powercycling the DDS16? I've seen it where a bad wire run shorts and sparks the port, rendering it unusable until I powercycle the module. If it's feasible, I would try re-terminating the cable on both ends, and if you still have problems, possibly a new wire run?
 
Check the cabling to that extension !
Did there perhaps was an S0 extension in the past ?
Ds cards and modules do not like terminators

The power builds up until the port blows up

When you power cycled the unit, make sure you unplug the port in question. Once it has powered back up, you can re-plug the wire back into that port.
 
Which reminds me, what type of phone are you using on the port that's having issues?
