DBM32 modules lock up in IPO 500 v2

We have a new client with an IP Office 500 V2. They were set up with a number of 1416 phones with DBM32 units so that they would have enough programmable buttons. The issue that they are seeing is that the DBM32 modules will occasionally "lock up" and light most or all of the buttons. The only way that they have found to deal with this situation is to disconnect the unit from the phone and reconnect. Sometimes they go as far as to unplug the phone.
Any thoughts as to the root cause?
A solution to this issue would be fantastic for them.

could be slowly dying on you, may need to replace, but as stated I would 1st replace the power supply and retest
 
I have had a customer like this

approx 9 users with DBM32

Replaced handsets (at least twice) DMB32's (at least twice) PSU's,
replaced ip500DS8 modules & combo Modules
Moved phone to external Module
moved to different patch cable
replaced IP500V2
upgraded firmware.

the only thing that ha not actually been changed is the user (Organic person Not IPO user & extn) who experiences the problem almost daily (company owner so unfortunately not an option)

Escalated to Avaya Via IPOSS who requested various traces (after first trying to suggest a handset fault & suggesting we replaced it !) & photos of affected phones so they could try to identify
Unfortunately we progressed no further as the customer ha given up paying attention to the issue & does not advise when it occurs.

Do things on the cheap & it will cost you dear
 
Extra power supplies were used for each of the DMB32's. They have been replaced as part of the troubleshooting process. According to the PoC, the issue is sporadic and only occurs after an active call at the affected extensions. It does not affect all of the DMB32's that they have, only a select few. All are configured the same.
 
What version? There is a limit of the maximum buttons for the whole system.
