CM error adding new mailbox

I'm trying to add a mailbox and keep getting an error that says Name or Touch-Tone equivalent already in use. Site administration let me add the extension but CM won't let me add the mailbox.

look in you mail system for the name already existing. Also look for a name spelled using the same digit numbers on the phone set ie T U V are all on the number 8 key.
