C.O. Call Waiting

Can anyone shed some light on how to make the Flash hook work in the following situation?
IP500 with analog 4 for incoming lines.

When the user hears a call waiting tone, he should be able to do a flash hook to put that call on hold at the C.O. and take the second call.
It actually takes 3 steps of flash hook.

You can generally assign hookflash to a button on keyset. Press the button yields flash to CO.

I have done this and it does not work
If I use an analog phone directly on the c.o. line then it works, but not through the IPO.

Adjust your "Flash Pulse Width" under the analog line tab. Careful not to make it to great or your flash will disconnect the call.
