BCM450 Services stopping

BCM450 R5 Several services stopped a couple of times today. IP phones couldn't connect to server was the first symptom. AFter restarting the BCM everything worked for a few minutes then the same thing happened again. I looked at the alarm log and here are some:

52005 *Error** Reset by Network Manager (many occurrences)
50 Core Telephony - A digital station module has become disconnected (three times)
53005 MGS::Shutting down due to MPS communications failure
50064 The media path management subsystem unexpectedly became offline - terminating
54005MRS::Shutting down due to MPS failure
10003 Service Manager - IP terminal services (NTPS) has stopped unexpectedly Service Manager is attempting to restart
10005 Service Manager - VOIP Gateway (FEPS) has stopped unexpectedly. Service Manager is attempting to restart.
50013 UTPS failed to obtain the detailed terminal list from the core telephony engine. The detailed error is : Framework Error: The FUMP channel is not available for sending data.

And many more. The I see alarms indicating the services are restarting and all services and phones are working again for several hours. Strangely, though, all but one of the T7316E set are seen as T7316/M7310 sets and the buttons are reset to default. I plan to replace the entire system ASAP but would like to know if any of you have seen this before.

I found there was a new SU just released. One of the issues it addressed was core telephony restarts. I installed it and the firmware was upgraded on the T7316 sets (a few)and all seem to be working as they should and are now identified correctly as T7316E. I'll monitor the system closely to see if any new issues arise.
 
Update - The problem came back this morning so I'm replacing the BCM this evening. I'll put it in my lab for a few days and see if I get any repeat failures.
 
I am having the exact same issue on a BCM450 6.0, services go down, phones flash, calls drop, then it recovers by itself.
These are some of the error messages:
50064 The Media Path Management sub-system unexpectedly became offline - terminating
54005 Mrs:: Shutting down due to MPS communication failure
53005 Mgs:: Shutting down due to MPS communication failure
43011 Lost connectivity to the switch
52004 **ERROR** FUMP channel not ready;MPS service aborted

Any ideas?

1.- Check AC Voltages:
120 VCA
ground to neutral < 1 VCA.

2.- disconnect lan cable, reboot BCM and check for errors

Supermario, I had all those exact same messages, then messages indicating all the services were recovering. I haven't had a single alarm since I replaced the whole box, minus the BFT, last week. I have the old one running on my bench but without much of a load.
morenot, I had just replaced the UPS a couple of weeks before the trouble began so checking the AC line voltage may be a good place to start if I see any more events. I'm leaning more towards a bad hard drive however, and that was replaced when I replaced the UPS for a different problem. I replaced it again when I replaced the box.
 
Same here today with BCM50 2.0 with latest patches.

I have a similar issue:

History:
BCM 450 R1 upgraded to BCM 450R5 had latested SU8 patch. Having many problems with conference brige issues.
Deceided to replace entire system. Backedup, restored into new system R5. Conference issues have been ok for the past week.

Today:
System locked up. All lights on BCM indicated ok. All digital sets froze with time and date. IP sets "server unreachable". 5 Mins later IP phones started to register system was up for 30 sec and went down again. Had to power cycle BCM.
Alarms received were:
10001 Core Telephony has stopped
52004 MPS Fump Channel
53005 MGS shutting down

System ok at this time. Any ideas??

Any help would be appericated..
 
This came back after several months of operating without any issues. Patches were up to date when it happened in January so I replaced the BCM and it's been behaving since. Anyone have any new ideas?

It would be good to see the time stamps to isolate what went down first.
Looks like a HDD issue.

The architecture of BCM450 introduced some negative changes.
No more MSC. That means everything is on the HDD, and if the HDD hiccups, it affects the whole system.
The MPS and MGS pertain to communications between the HDD and the telephony. When these services stop, its a major problem.

Watch the Patches. If the system is working OK, you don't really need to install them, but at least look at the ReadMe files to see what types of issues they deal with, it may just be a matter of time until one of your systems is affected.

The latest SU read me refer to MPS and other services stopping being fixed but those were in the section referring to the prioe SU which is already installed. I've erased the logs so no time stamps are available. I'm going to go ahead and install the latest SU when I get a maintenance window and also check the hard drive ATA connector. It's been running without errors for a week now.

Everything, including the hard drive, was replaced except the main board after the first time we had this issue.
