BCM450 on PRI, Fax errors "Poor Line Condition

I have a BCM450 R6 that is fully patched. We are using the CallPilot fax for desktop faxing. Off and on we have experienced problems receiving faxes. People trying to fax us get an error on their fax machines that reads "Poor line condition." Recently this problem has grown and now we have several of our remote offices that cannot fax to us. I had had the local telco check the lines at the office. I have had our PRI provider test the circuit (voice PRI, not IP). No problems are found. Both are telling me it is the BCM450. The PRI provider says everything tests good and that when they try to send a fax to one of the DID numbers that are fax lines, they get a time out error. What could be wrong with the BCM450? It is less than a year old.
 
The clock source on the BCM450 is set to Primary External.
 
I will list out all the settings:

PRI comes in on a T1 card that is connected to an Adtran 1203022L1. The Adtran's DIP switches are set to: 0dB Attenuation, 0-133 feet cable length. The Adtran is connected to the DTM-PRI that is installed into the BCM450.

The DTM-PRI settings:
Trunk type: PRI
Protocol: NI-2
NSF extension: None
B channel selection: Descending Sequential
Clock source: Primary external
Send name display is checked.
T1 Parameters:
CO fail: TIA-547A
Interface levels: ISDN
Framing: ESF
Line coding: B8ZS
Internal CSU is checked
CSU line build: 0dB

 
How are they delivering the PRI? If they are using VOIP and then breaking it out as a PRI there could be an issue. They may need to set all the channels for voice and data.

Deltacom did not have IP based PRIs when this one was installed here. It is not VoIP. I have verified this with Earthlink (they assimilated Deltacom).
 
Check to see if the Adtran is also a CSU because if it is and it is turned on that could give you some problems. I've issues in the past with an External CSU connected to an MICS with the Internal CSU turned on.
 
I was curious about that as I know the Adtran is a CSU. Just unchecked it to test it out. I will post an update after testing. Voice calls worked fine still after unchecking it.
 
That did not fix it. I also have reports that when faxes come through, random pages are missing from the fax. Some pages are streaks of black lines on them.
 
I had a tech from my vendor and a tech from the PRI provider both here to fix the issue and they could not solve it. They are claiming the problem must be at our remote offices. Trouble tickets opened everywhere. I will see how this plays out.
 
I am familiar with dirty scanners on fax machines. These look more like something messed up during transmission of the fax. It is coming from multiple senders.
 
Have you tried taking the Adtran out and just using the BCM's internal CSU?
