Make sure the CO is sending supervised disconnect. Thats missed quite often. If you have Mer Mail forget COT it will not work. GRD or T1 is the only way.

If your CO provides Loop Start trunks then look at BTID settings for one of your trunks that does disconnect. Once a trunk has a truk built on it and a BTD has been set then all the other trunks on that card get the same BTID. If this is new card/trunk install you will have to test different settings until the call breaks down from the far end "hanging up".
