All phones showing unplugged? 1

I've got an IP 500v2 with a combo card and a ds1-8 with a PRI card on board that is currently experiencing this issue. PRI us up and when I log into system status I can see all the calls coming in, but when I click on extensions they all show as unplugged. Had customer plug a digital phone directly into port 1 of the combo and got the same result. Currently upgrading from R9 sp 3 to sp 4 in the hopes that will resolve it. Any other ideas?
 
This happens when someone plugs a PC/switch connection into a digital port, if this is the case the port is likely fried...unless you're lucky :)

 
I'm thinking the issue may be that the Combo card is in slot 1. That's been something I've found to be problematic in the past. Going to have my guy swap the cards once he gets to site.
 
Guess I am lucky in another way then. Thanks for the info. :D

As you probably guessed, moving the cards around did nothing.
 
Have you unplugged all the handsets? If 1 comes up add another etc etc :)

 
Does system status show the cards as properly showing up? I have had (2) IP500V2 within the past week that the combo shows as "invalid daughter card" in system status. In both cases I had to replace the 500V2 cabinet. Both brand new installs at latest R9.
 
Yep, cards are displaying properly. Nothing is showing invalid in the system status. Looking in system monitor it doesn't appear that anything is trying to upload. The phones aren't powering on at all. Headed back to site with a new KSU now. Will let you guys know how it goes.
 
Plug in one phone/lead at a time starting with an empty chassis, see which one breaks it...or don't any work? :-

 
I'm on phone with Avaya and he wants me to rebuild the SD card. Heh.
 
Doing the one by one thing now. I had already tried it with just the DS8 / just the combo in.
