All Ciruits Buzy on BCM450 PRI

I keep getting inbound complaints of "all circuits busy". I have had Telco test, and test, and test for weeks. I replaced the DTM card, and checked settings over and over. It seams that the system will not grab channels properly. Is it possible that the BCM is causing this issue? I am about to put a ticket in with Nortel, but I feel it is a telco issue still.
 
And you have seen this while on the monitor? Are all lines tied up , what is the PRI set up as?

is is possible they are getting this on a DID call to a user who is already on a call ?

check the target line if so.

But it sounds like a telco issue.

It's only dialtone-VZ
