Agent gets a dial tone after releasing a call!

Sometime, when an agent tries to release a call, instead of going to after call work, it goes ready, and he gets a dial tone!!
Any ideas what is happening here?

OXA 2.5.14
CM r8.1

Thank you Avayaguy23 for your response. the thing is, that I cannot reproduce the issue because it can happen once a day or a week :)

You are running OXA in i'm assuming soft phone mode. The debug logs should be able to tell you.

Great Idea! what level should I set them to? Also, should I enable both UI and SparkEmulator logging?
