Hi, I've never used Premiere Pro, but when exporting images without a background to Quark this is what I do:
Select the image you want to be on transparent background (make sure that the layer you are working with is not Background)
Go to - Help - Export Transparent Image
Then, just follow...
Hi, I think it's possible that the problem is in your PDF Settings.
When you go to export as PDF, go to Option - Job Options and make sure that Subset font option is unchecked and Embed all Fonts option is checked.

I wish I've seen your question earlier.
I work for a prinshop and we see this kind of problem all the time. What we usually do is PDF the file, then us the QuiteImposing plug-in which paginates books. The whole process takes about 2 minutes.
Anyway I hope your book is a huge success.
