Are you seeing this error on the CMS server or a Windows based workstation? schmdt is a shared memory reference and IPC is most likely inter process communications.

Do you use ASA? If so, look under the the following folder:
C:\Program Files\Avaya\ASA\data\setimage

We have this happen quite a bit. Usually it is with users with laptops, and for some reason, the laptop hibernated while CVS was up, and they rebooted the machine to get it back. It seems like the problem happens when CVS is running and the workstation or Laptop is then shut off abruptly.
If you assign your mailbox a COS that has broadcast permissions, and change your subscriber account to allow broadcast and you should be able to send a broadcast message. Look up the broadcast feature in your AUDIX documentation and it should explain it in more detail.
