At first I cured it by installing the modem in another ISA
slot after REMOVE. It worked OK for a couple of sessions online then the problem recurred. I changed the slot again, but it was not a permanent fix. The last fix was merely REMOVE / REBOOT / INSTALL...

The same thing happened to me. The program name just
disappeared from the list. However, I do not have TweakUI. I can still access it through Find. I never did figure out how to put it back on the list. Nothing grayed out when I right click START.
