I have a site where the NXX is 775. There are a few ranges of DID that we buy from the telco in the 4000 and 7000.
Curiously enough, I noticed that in the LUDN (snippet below) there are some listings of the NXX-DID giving a full 7 digit.
Can someone explain why there would be 7 digit...

We are having a bit of an issue with our call pilot. We recently upgraded from CP 4.0/PBX 3.0 to CP 5.0/CS1000 7.5.
We had the same intermittant issues before the upgrade but seem to be a bit more common now.

opt-11 rls3 with TIE to 81C rls 7.5.
TIE programmed with 1 DS0 as DCH.
All DID delivered to opt-11 via the TIE 81C (CP mail located at the 81C end for the opt-11)

You may want to look into the Call Fwd External feature. When allowed, the user can get a dial tone by the method you described. In my system, all users are denied unless specifically requested.
