You may have a loop in your network that is bringing the switch CPU to it's knees. You could probably connect a laptop to any of the switch ports and run WireShark to see if there are any loops/broadcast storms.
You need to either configure the phone through the handset (WLAN ESSID, Security, etc) or you need to connect the handset via USB to a PC and use the Handset Administrator.
It sounds like you've completely skipped configuring the handset with the necessary WLAN information.

It might be a code issue with the BayStack products but definitely not the Passport (Ethernet Routing Switch).
If it's possible I would setup a sniffer on the edge switches to confirm that your telnet (TCP/23) packets are actually making it to the switches.

HmmHmm... that's an interesting problem. You mention "password" changes but it sounds like you were previously using RADIUS authentication?
What version of software are you running on the 470s?

This could be a classic ARP/FDB issue with your core... what version of software are you running in your core? If you dump the ARP and FDB tables from the core switches do they point to the SMLT port that connects the edge switch or do they point to the IST ports?
It could also be a MLT...

It looks like v3.7.3 was pulled from Nortel's website. You'll need a support contract in order to download any of the currently available releases. I would stick with 3.7.2 although I'll need to inquire as to what happened with 3.7.3

I believe the latest version of software for the ERS 4500 series now supports ADAC but not sure about the ERS 2500 series. You definitely need the ports configured as untagPvidOnly of the PC won't know what to-do with the 802.1q tagged frames. You didn't mention if you are trying to use LLDP-MED...
There was a bulletin posted to Nortel's website concerning a port bouncing issue that was resolved in 5.0.1.0 software.
I believe they are still working on the lane lockup issue but are hopefully to have it resolved in 5.1.0.0 software...
Well 4.1.8.2 has been rock solid so I'm hoping that 5.0.1.0 will pick-up from there... it took them long enough but I hopefully that things will be very stable.
Are you using the latest and greatest firmware for the 6140?
We had a voice quality issue using the latest software release for our Motorola Wireless LAN switch until we upgraded to the latest release for the 6140, I believe it was firmware release 115.019.
