Why flatten the image in the PSD?
If you want to work on a flattened version of the image, select your top layer and press 'ctrl + alt + shift + e' to get a flattened version of your image on top of the other layers on which you can make the specific changes.
I've used appzapper myself, works like a charm.
It automatically finds all the preference files that come with the installed application and 'zapps' them all at once.
After a fresh install Photoshop can't open the RAW's from your camera. Download a RAW update from: http://www.adobe.com/products/photoshop/cameraraw.html
Since there are new cameras coming out all the time, they release updates every now and then.
Or just make a selection with the rectangular marquee tool and go to image -> crop.
You can constrain the aspect ratio etc. like you normally do with the rectangular marquee tool.
My first guess was that you have 2 prompts, cognos treats the values between question marks as prompts.
Your method of adding a prompt looks good, though I don't see the added value of dialing in the display value, since you use the same value for it.
Check the query to which you added the prompt, in the top right corner there is a 'Detail Filters' section. Check to see which values are between question marks and delete the one you don't want to be shown.
You probably have two ?values? between question marks.
