Why would you want to have a manual failover? Return from ESS mode in a manual way I can easily understand but why make your resilience a manual operation?

Personally, I'd use a NTP source rather than rely on VMware to provide time services, but it's a bit late for that advice
Communications Manager will not do this out of teh box. You'll need either a dedicated product (and I don't know of one) or an IVR system such as Avaya Aura Experience Portal (AAEP does include a SMS module) and SMS carrier connection etc.

Does fault follow handset? if yes, either faulty hardware (4602 hasn't been available for 15 years or so. it may just be dead) or config. if no, then maybe network (cable, switch port etc)

If you have a 3rd Party cert on your SMGR and you don't have the root installed on the SM/BSM then you are going to have issues. If you need to change the hostname then run SMnetSetup scrip too

Watch out for ARP issues! You'll now if you have them as there will be speech path issues. You'll usually experience then if the phone changes IP address. Its not usually an issue but in certain break-fix scenarios, who knows!
