I think it's the default setting, but double check in microsips settings that dtmf is set to rfc2833, and in the voipdb dtmf settings, that profile 1 for sip extensions is set to rfc2833.

Extensions that arn't connected can't be programmed. In PcPro, you can make them programmable by going to the Filter Options tab, clicking "IP Phone list", selecting the extension extension number / port, and ok.
