Thank you for bringing this to my attention. I spent all day tracing cables on a completely unlabeled patch panel, with no rhyme or reason as to what went where, in an 80 degree room. Finally got OUR stuff separated and labeled. Had an old IP400 DS30 and plugged that in, plugged everything...
Our vendor support suggested upgrading the IPO it connects to from 9.1.11 (9.1.8 in the case of my test box) to 9.1.12 to see if it resolved the issue.
Fix suggested above did not work. Tracking down a serial cable now...it may have to wait till tomorrow as I will have to grab mine from home...
Just got off the phone with our vendor support. Apparently the version of our DS30 needs 9.1.12 to run right. Trying it out, will post results.
Edit: Blowing up may be a bad choice of words. They give a red light, blink back to green about 30 seconds, then blink amber to red, then back to...

I have an IP500 V2 connected to 6 DS30 Expansion modules. I have replaced 4 DS30's over the last month and this particular IP500 seems to just burn out each new one I put in. They all run off the same ground and same surge/battery...
