Writing a proposal?

I recently resigned from a company (moved out of state) where I was the network administrator. While I was there, I created some access databases, just for fun, as learning tools. The company implemented a few of them and it was fairly successful. Now they want me to do some work for them creating more databases and have requested a proposal from me. First, I have no idea how to write a proposal. Second, I don't know what to charge. I mean, I worked there, they know what my salary was, but since they're not paying benefits and I'm not an employee, I'm not sure what's fair as a contractor. Also, I'm far from an expert, just self-trained. Any advice you could offer would be greatly appreciated! Thanks.
