where to start with IP phones?

We just got licenced to run IP phones on our CS1000. It is on release 7.5 and I purchased a i2004 phone to start testing with. We have separate VLANs on our network for Data and Telephony traffic.

Can someone point me in the right direction to where I can find some steps to get off the ground?
I have no idea where to start. I tried building a phone in the system and putting that particular data connection on the correct VLAN, but had no luck with it communicating with our phone switch.

I have tried it both ways...static and DHCP. Neither setups give me any type of result.

What would be my best bet for factory resetting?
I see you can push **RENEW then my MAC address followed by ## ...that doesn't seem to cause the phone to do anything.

Also, is there a way to identify if my i2004 is a phase 1 or phase 2?
 
The "reset to factory" does not work on a the i2004 - which was discontinued over three years ago. A phase 1 i2004 is NTDU82. A phase 2 i2004 is an NTDU92.
