What info does the Telco give me and what info do I give them?

You tell them what FR parameters you require for provisioning.

Port size
CIR
T1 provisioning (B8ZS, ESF, AnnexD, etc)

That's about it. They will give you a DLCI number for the PVC that each side needs, and that's it.

You can also request the dlci numbers used in your frame relay network. It can help keep the information logical with multi node networks. with just two sites it would not matter and the telco will probabley map dlci 16 to dlci 16 in that situation. If you can get B8ZS and ESF. Mainly so that the telco can test the line with out droping it.

It is generally best to sepcify the DLCI request. As well as the line size and the amount of data pass thru you want. 64k Circuit with 32k CIR will give you a 64k circuit with a tru put of 32k at any given time, you can also specify the burst rate if need be, the only purpose for line code and framing (B8ZS/ESF) is if you are ordering a full T-1, even then the telco has to take the line down for testing. They can monitor the line and the packet transfer with out taking down the line no matter what you are using.
they inturn give your circuit ID, demarc information and the DLCI confirmation or changes made to accomodate your presence in the frame cloud itself.

Im a Telco guy, I can get you where you want to go, but I can't guarantee it will work when you get there!
