What happens the first time? Do you get a message on the screen or an error/engaged/etc tone?

no errors coming on screen, it will go back to the normal screen after i dial the number.

Monitor! That is what the app is for.

No sound either? Just instantaneously back to the main screen? And you dial the same number again and it works? Is this consistent, the first call fails, the second works , the third fails, the forth works etc, or is it intermittent? What does it show if you monitor the extension on System Status?

Are you clicking redail for the 2nd 3rd and 4th attepmt as it maybee the user is too slow dialling and you may need to alter the dial delay count!!

how many trunks do you hAVE? go to system status and monitor the trunks, you should be able to identify which trunk is not connected

It sounds like line id's which are the same for trunk port in use and not in use.

BAZINGA!

I'm not insane, my mother had me tested!

As I said, use monitor to see what happens, the phone screen won't show anything helpfull.
