Weird issue, single call on 2 lines

I got a call regarding a weird issue that occurred late last week at one of our sites. At the site they have a BCM450, with IP phones and 5 analog lines from the telco. Here's what they(the people at the site) said happened.
* a single person called in through the outside lines, 2 of our outside lines rang ie ###-###-9900 and ###-###-9910 at the same time. 2 different internal people picked up the 2 lines and talked to the same person and could hear one another. This happened approximately 3 time on 1 day and hasn't happened since.

Any ideas?

Sounds like classic cross talk.

A good test is to ANAC the lines too.....if you here the read back giving you 2 phone numbers at the same time then chances are the pairs are mixed up.

I see what you're saying, however if it was cross talk wouldn't it happen all the time on our lines? It's only happened 3 times and only on 1 day.

Was it from an outdial on a BCM as well. I've seen this caused from BCM's. It usually results in unsupervised conferences that lock up two CO lines rather than two calls to the same number.

Caller dials number 9 XXX-XXX-XXXX and thinks call wasn't completed, rather than hang up or release call, they press transfer and they dial again and then conference two outgoing calls to the same number.
