VPN phones over windstream internet showing discover x.x.x.x

I have a customer using 10.1. They have 4 VPN remote workers. 3 of the 9620 VPN phones I sent out came up no issue. The 4th one didnt come up at the users home office. I can get the phone working on Comcast and AT&T but when I take it to the customer that has windstream the phone connects to the VPN but shows discovering x.x.x.x
Has anyone one else had an issue getting a VPN phone up with windstream or any other vendor? If so what was done to get them working. I tried forwarding ports 1719 & 1720 along with a few others. Anyone have a full list of what ports the vpn phone is using?

Is the phone behind a firewall ?

disconnect phones, make a base line trace using wireshark, compare the trace to the faulty phone.

I've had many issues with carrier provided routers and vpn connections for phones. Make PPTP/L2TP/IPSEC and H323 passthru is enabled on the firewall.
Sometimes it is, and then it's a firmware issue on the firewall. We've had to go in and put customer modem/router combos in bridge mode and use some other firewall (like netgear, etc) to make it work.
Also, sometimes the IP Office needs a route back to that subnet. IE, the customers home network is 192.168.50.x, but the ip office does not have a route back to the VPN device for that subnet.
