Voicemail with mind of its own

One of my users is complaining that if he is on the phone and a second call comes in, when it goes to cover it sometimes stamps the voicemail as being from him, the receiver instead of the actual sender. In other words, when Joe Blow answers his voicemail it says, "you have a message from...Joe Blow"

I've checked the coverage paths and voicemail subscriber configurations and can't figure it out. Any ideas? We have the Definity system with Intuity Voicemail
 
What type of vm system are you running? What version? What's the integration?
 
We are running Intuity Audix ver. 5 with Definity G3si PBX
 
Make sure that on his station form "LWC Reception" is set like all of the other stations. Other than that, I can't think of anything that would make that happen. It's quite strange.
 
I'm thinking that his admin may be picking up the calls on a bridged appearance and tranfering them to voicemail.

Isn't there a way to tranfer or forward calls to voicemail that would cause it to stamp it with the extension called?
 
Could the coverage path have active set to Y? If so the next call would go immed to VM.

The active settings is set to n for outside and inside calls. I'm thinking it was the admin's error. She was tranferring people to voicemail using the abbreviated dialing button for voicemail. She would hit transfer>>enter the extension>>abrv-dial btn>>transfer. For some reason it was freaking out when she did this. I'm still testing but I created a transfer to voicemail button for her so that she doesn't have to enter the extension.

I'm thinking that if she answers a bridged call and enters the extension of the line that she is answering on, it may be causing the stamp. But I'm not 100% sure, I'll give it a few days to see if the behavior reoccurs.
