Vociemail login not working after upgrade. 2

Customer was on R7 with VM Pro.
Swapped out IP500v2 for a v2a.
Upgraded to 11.0.4.4 via PLDS with new LAC and IPOSS.
But IPOSS won't kick in until sometime in December so I'm asking here for help.

Two error messages, neither seems to be service affecting but do I need to actually do something about them?
"Combo cards not supported on this platform"
"A valid Software Upgrade license must be configured for this version of firmware"

Vociemail and auto-attendants appear to be working properly except...
1408's and 1416's get "invalid login" after dialing extension # or #.
1403's work as expected with no issues.
Can login to a 1408 or 1416 voicemail from a 1403 set.

Any ideas as to what I need to look at?

There is a solution on avaya for it

It is to do with the incorrect firmware on the handsets. Gives double dtmf
Need to upgrade handsets twice using no user codes

The links to bin files in the kb doc don't work, but i was abel to find the file by searching 1408 on the support site.
 
IP500V2A needs 11.0.4.8 or any 11.1 version so if you upgrade the error message for the combo cards will disappear.

I think if you trace the 1400 phones you will see that it sends all digits twice, that will need the updated firmware that daken posted with the SOLN325073 document
