Very Rare High pitched tone that kills a call

Hey guys; - Running IPO 500 v2 8.1 9611 phones

This is an odd one I have here.. and unfortunately I don't have a trace because this happens so rarely I would have to run a trace 24/7 to catch it.

While on the phone one our users experienced a high pitch tone (described it as similar to emergency broadcast tone) it killed the call and the users screen saver came on (usually takes 40 minutes).

I'm not sure what could cause something like this, whether it's a setting on the IPO or if it's our provider that needs to tweak something. This happens about once every 3-4 weeks.

Any help or pointing in the right direction would be appreciated.

I would say it's the handset, something similar happened on digital sets a while ago, but that was a hardware issue. Also the fact this effects the firmware on the phone by bringing on the screensaver points to the phone itself :)

What kind of voice service are you using?

We had an event like this yesterday, in fact, in the southeast US. To make a long story short, it was a carrier problem. AT&T provides voice and internet service to the customer via a T-1 and provides voice channels to the equipment through a channel bank. Incoming callers to one of the lines heard loud electronic interference. Outgoing service on the line was unaffected. As this installation is less than a month old, the customer was persuaded by AT&T it was a premises equipment issue - but after our tech reset the channel bank (twice) the noise disappeared.
 
Now that I've re-read my post, I would have to agree that AT&T was correct - only it was their premises equipment, not the Avaya system.
 
In our case this was an outbound call, and I think the majority of times it's happened (although rare) it has been on an outbound call.

I guess my next step would be to run the sip trace to see where the disconnect comes from so I can cut the problem in half and figure out if it's the provider or the IPO.
 
If this only happens to one phone only then I would suspect a problem with the phone itself, replace phone and retest.

It's not only one phone. It happened to a conference phone we have and also a 9611 phone.
 
driven, your post does not state that so that is why I responded the way I did, I would have your circuit tested and go from there.
