Vector randomly going to hold music

I have a call tree built... In the vector there are no instances of going to hold music and it shouldn't be doing this. While I can't recreate the issue myself I have reports that sometimes at random when calling the main # they go straight to hold music and stay there. Anyone have any idea what could be causing this?

Things I have confirmed it not to be.
It is not a time of day issue
Its not a queuing issue.
 
Make sure you have a wait 0 sec hearing ringback in the vector before any steps as a fail safe. That might help.
 
What does the vector look like?
If it is a vector that queue to a skill, you could possibly have an agent that is answering the call and immediately putting the caller on hold.

Can you verify the are actually calling that main number? Maybe check all the vectors where that announcement is being played.
 
How about the hunt group? Is the queue number high enough? Have you tried tracing it? list trace
 
Could it be that there is an agent who puts the call straight on hold and then sits and reads the paper [ponder]

no, because when you first call in it goes to a vector that is just an announcement and then some goto's there is no queuing for agents until they press 0 for the operator. and according to the operator manager he tried calling in and didn't even get the Welcome message just right into hold music which I'm kinda finding hard to believe... I'm starting to think i'm not getting the full end of the story =)
