Users name not Displayed

I have a Nortel 1230 phone that will not display internal incoming caller names only the extension. Also if a call is make from this phone only the extension shows up on other phones. Am I missing or need to remove any cls codes. Note the TN info below

[GHTROUT]

KEY 0 and 1 are the same to conserve DNs (note they are MCR). It allows calls placed from either key to show the same DN, and you can juggle two incoming calls.
