Upload servlet

I need an upload servlet for my project.
Does anybody know where i can find a complete example of a upload servlet and a belonging html form for calling it.

org.apache.commons.fileupload

If you google, you will find exactly what you are looking for. Just remember not to code all that into your JSP though.
