upload file to a database with asp

Hi, can anyone help me to create a code to upload a file from a local computer to the web and put it in a databse?

Do you really need to upload the image into the database or could you just upload an image to a folder and store the information about the image in the db?

Thanks for your help, I just need to upload a file to the server and then store the information in the db
the files are all kind of images (bmp,gif,tif,jpg)

Well I tryied to use it and it works but it doesn't upload to a database and that's what I need.
One more thing is to upload a file like you gave me and then create email with CDONTS but when i'm trying to do that it says that request form can't work with that upload files. Any suggestions?
