Update Trigger Write Conflict

I have an update trigger on one of my SQL tables. In the adp client there is a form that is bound to the table. Now since adding the trigger, the form gives me a "Write Conflict" error whenever I try to update any information on the form. If I remove the trigger all works fine. The trigger makes an update in a different table so I don't know why is there a write conflict.

Any ideas??

The first table is my standard table the second I am using for audit history. The trigger sits on the table tangram.tblIssueLog. The field a_issueID is not an identity field and is used to store the issueID from the main table. I am not currently running this through a stored procedure just on the table with a trigger. The trigger does fire however an error occurs:

"The data was added to the database but won't be displayed in the form because it does not satisfy the criteria in the
underlying resource"

I did not understand this because I am not entering through a form but the table itself.
