Unable to add an extension

Hello everyone!!
Yesterday encounter a situation:
Mitel 6.0 new digital extension all the way, I can not add!!

SYS Logs: DBASRV: write_view_tuple error occurred but not reported for view 191.

This moment can only delete the extension could not be added!
But 2H then returned to normal!!
Will someone come across the same problem?

The system wasn't able to write to the database for some reason, possibly there was something or someone else trying to write to the same DB table at the same time. Did the next scheduled DBMS check complete (runs at 5am by default) successfully?
