UCM reimage error 2

I had a failure with the UCM 9.1. I started the reimage process using the Avaya docs.. I attached a screen and keyboard and when I went to start the process I get this error screen after it finishes the part where it boots when the long status bar at the bottom finishes ( screen shot below ). I checked the troubleshooting docs and the web and this site for any reference to this error but found nothing. Any suggestions? Thank you.

UPDATE: Turned out to be a faulty USB drive file. When that was fixed reimage went smoothly.
